Am I having an Orthodontic Emergency?

It’s rare that an orthodontic issue is an emergency, but when it does happen, it can be hard to determine if you need immediate care. Sometimes wires and brackets become loose and cause irritation, and occasionally pain, in the mouth. If this is the case, we recommend contacting your orthodontist as soon as possible during business hours. However, if you are suffering from an injury that’s causing continuous, sharp pain, it’s important to call your doctor immediately. Fallen teeth, though rare with braces, or a tooth that becomes loose, are also orthodontic emergencies and you should contact your doctor as soon as possible.

Minor Orthodontic Problems

- Sore Teeth: During the first few days after braces are first applied or adjusted, it’s common for teeth to feel sore. Eating soft foods, rinsing with warm-salt water, or taking over-the-counter pain relief can help.

- Mouth Sores: Another common issue is that braces can rub against the inside of your lips and cheeks and cause sores. The best solution is to use a bit of orthodontic wax to cover the parts of the braces that are rubbing. If you don’t have any, it’s available at many drugstores or you can also ask our office for more!

- Broken or Loose Braces/Brackets: Eating hard foods can cause a bracket to come loose or even pop off. If the bracket is still attached to the wire, leave it in place. If uncomfortable, cover with wax. If it comes out completely, contact our office for an appointment and bring it with you.

- Poking Wire: If a wire has come loose, sometimes it is possible to gently push it back into place with a Q-tip or a clean pencil eraser end. Poking wires can be covered with orthodontic wax, and, as a last resort, wire ends can possibly be clipped with sterilized clippers.

- Loose Teeth: During orthodontic treatment, it’s common for teeth to feel loose due to the gentle pressure and force of the braces shifting the teeth to a new position.

True Orthodontic Emergencies

- Mouth or Facial Injuries: Impacts to the mouth or face can result in cuts from braces on the lips or cheeks, broken brackets, or snapped wires. These injuries often occur from falls or sports accidents, like being hit in the mouth with a ball (which is why wearing a mouthguard is essential!). If you experience such an injury, contact us or your dentist immediately. An X-ray may be needed to assess the damage and determine the appropriate treatment.

- Shifted or Broken Teeth: Severe mouth injuries can sometimes cause teeth to move out of place or break, and having braces can complicate the situation. In these cases, an oral surgeon may be required to treat the teeth, and once addressed, the orthodontist will adjust or replace brackets and wires to realign your teeth.

- Swollen or Infected Gums and Teeth: Gum or tooth infections are serious and need prompt attention. These can result from poor oral hygiene or even allergies. If you suspect an infection, let us know, and we will guide you toward the proper treatment.

- Severe Pain in the Mouth, Teeth, or Face: Persistent severe pain could be a sign of injury, infection, or another issue. If you're experiencing ongoing pain, seek immediate evaluation to identify the cause and get appropriate treatment.
